The online UI Initial Claim application allows unemployed individuals to file new or additional claims and to reopen existing Arizona claims for unemployment insurance via the Internet.

IMPORTANT NOTE: The Internet UI Initial Claim application is available between 12:01AM Sunday until 5:00PM on the last working day of the week (this is normally Friday, except for state holidays that happen to fall on a Friday).
 
When you apply for Unemployment Insurance Benefits, you are required to verify your identity. Additionally, the Arizona Department of Economic Security (AZDES) will verify your Social Security Number through a computer match with the Social Security Administration. If there is a question about the Social Security Number you are using on this application, you will be notified in writing. We cannot pay benefits until the matter is resolved.

 
Although federal and state laws prohibit the revealing of information about your unemployment insurance to your spouse, relative, friends, non-interested parties and private interest groups, federal legislation requires that such information will be used for other governmental purposes including verifying eligibility for other governmental programs. Confidentiality will be the responsibility of all agencies using the information. AZ DES uses a debit card system for payment of Unemployment Insurance (UI) benefits—paper checks are no longer issued. The Electronic Payment Card (EPC) is made available through Comerica Bank and provides a safe and convenient way to receive weekly UI benefit payments. If you previously were receiving benefits by debit card you will continue to receive payments in the same manner. However, if you previously signed up for direct deposit to your checking or savings account it will be necessary to complete another application if you have not filed a weekly claim for more than four weeks and your benefit year has not yet expired.

When you complete and certify your application for UI benefits, you will be authorizing the AZ DES, Employment Administration to make automatic deposit of the full amount of any UI weekly benefit payments you are entitled to an Arizona EPC.

Before filing a claim, you will need to have the following information available:
  • Your Social Security Number

  • AZ Driver's License or AZ State Issued ID (Note: you may still complete your application without these two items).

  • Your mailing address, city, state and ZIP code

  • If you live in Arizona, know the county in which you reside

  • The names, addresses, and phone numbers of all your employers for the last 18 months

  • The last day you worked immediately prior to filing your UI claim

  • If you received severance, lieu of notice or dismissal pay; payment for unused vacation, sick, or holiday leave; or payment for military accrued leave--the amount (before deductions) and the date it was paid to you

  • The name and local number of your union hall, if applicable

  • Your Alien Registration Number if you are not a citizen of the United States

  • Copy # 4 of your DD Form 214 if released from the military in the last 18 months

  • SF 8 or SF 50 if employed in Federal Civilian service in the last 18 months

  • If you are collecting a pension (OTHER THAN SOCIAL SECURITY), you will need the start date of the pension and the monthly benefit amount.
 
If you are unable to locate the above required information within the first week you are unemployed, call the AZ Call Center so you can claim this week. Delay in reporting will affect your benefits. Click Here for the Call Center Phone number.
 
Warning: The last page of the Internet application will state that your claim has been completed and will give you a confirmation number. Unless you receive a confirmation number, your claim will not be processed.

If you exit the application before receiving a confirmation number, any information that you have entered will be discarded.
